A Form can be used to collect multiple kinds of information from the learners once they enroll into a course.
Example: After completion of a chapter, you can roll out a feedback form asking them to rate the chapter content on various factors such as "easy to comprehend", "worth for money", "Quiz Difficulty" Etc. 

Steps to Create a Form:

Open your course in Course Builder and click on Create New Item. Click on "Form", fill in the title of the New Form, then Submit to get started.

You will be redirected to an HTML text page where you have to enter the Form details along with its title. This feature allows multiple types of texts including special characters.

Title: This contains the title of the course as entered by you while creating the form. This field is editable.

Checkbox

The checkbox is shown as a square box that is ticked when activated. Checkbox allows the users to select one or more options of a limited number of choices.
Example: If you want to collect data on the number of sports played by your learner;
Which of these sport do you play actively:
Cricket
Hockey
Basketball
Volleyball
Football

In the above question, there will be a checkbox beside every option. The learner can select the multiple numbers of options as per his answer.

To include a checkbox in the form, click on the checkbox symbol as marked below. 

Name

A "Name" is the batch question title under which all the checkboxes of the same question will open. Please note that the "Name" is not visible to the learners & is only for the internal purpose.
Fill up the "Name" under which the checkbox will open. 

Value

This is the pre-filled answer. In the checkbox question leave this field blank.

Selected

Keep this option empty for the learners to select.

Required

If you don't want your learners to skip this question then check this box.

Select "OK" to submit the question.

A Checkbox will appear on the form section. 

Place the cursor beside the checkbox & enter the option Text.
Repeat the process to place more Checkbox.

Click on "Save" to save the above changes.

Radio Button

Radio Buttons are typically rendered as small circles, which gets filled or highlighted when gets selected. It can be used in a group question where the learner needs to choose just one of the options.

Example: What's your gender?
Male
Female

In the above question, there will be a radio button beside every option. The learner can only choose one of the options as per his answer.

To include a checkbox in the form, click on the radio button just placed after checkbox.

You will be required to fill in the below fields.

Name

A "Name" is the question title under which this question will be recognized by the server. Please note that the "Name" is not visible to the learners & is only for the internal purpose.
Fill up the "Name" under which the text will open. 

Value

This is the pre-filled answer. In the radio button question leave this field blank.

Selected

Keep this option empty for the learners to select.

Required

If you don't want your learners to skip this question then check this box.

Select "OK" to submit the question.

Text Field

The Text Field is used to gather information which involves smaller texts.
To include text questions in the form, click on the "Text Field" placed just after checkbox.

Name

A "Name" is the question title under which this question will be recognized by the server. Please note that the "Name" is not visible to the learners & is only for the internal purpose.
Fill up the "Name" under which the text will open. 

Value

This is the pre-filled answer. However, learners can edit this as per their need. Leave this field blank if you want the learners to type in their individual answer.

Character Width

This determines the size of the characters entered by the users. 

Maximum Characters

To restrict the length of an answer, set the maximum permitted limit of the number of characters used. The users won't be able to write an answer beyond the permissible character limit.

Type 

Select the type of answer you want the user to provide. The provided types of questions are Email, Password, Search, Telephone Number, Text & URL.
Please note that the default setting of this field is "Text".

Select "OK" to submit the question.

Text Area Properties

Text Area is used to gather descriptive text like an essay. This field is draggable which makes it convenient for the users to type in multi-line text.
To include Text Area in the form, click on the "Text Area" placed just after "Text Field".

Name

A "Name" is the question title under which this question will be recognized by the server. Please note that the "Name" is not visible to the learners & is only for the internal purpose.
Fill up the "Name" under which the text will open. 

Column & Rows

The size of the text area can be specified by rows & columns.

Value

This is the pre-filled answer. However, learners can edit this as per their need. Leave this field blank if you want the learners to type in their individual answer.

Required

If you don't want your learners to skip this question then check this box.

Fill in the details, then click "save".
